There is always sledge hammers and lump hammers to break it up but a decent sized Kango would be a lot easier. If you are doing a path that size a concrete mixer would be best. That's what they are made for. You could have it tarmacked over providing there was no heavy items going on it. Unfortunately you can't make a silk purse out of a sow's ear!
Edge down both sides with a nice pave brick, and then in-fill with a decorative gravel. Go for a nice contrast - say a black pave with white or Cotswold chip (white or buff). (No Kangos, no sledge 'ammers, no mixers - just hand mix a small batch of mortar to lay your paves...)
The wooden door threshold is already low, taking into account the thickness of the slab and the bedding. The best option is to kangos the lot (upto the neat concrete slab), and start again. Be extra careful around the clay collar for the soil pipe. Also is the a manhole cover4, in which case buy a suitable tray cover. Set it out so there is a fall in the paving towards the fence panels, ideally may consider putting in a narrow border for plants and shrubs.
The wiring is rough as well - is that telly downlead or some decorative brown flex borrowed from an electrolier to run the garden shed?

Depends on what waste, is it just for a boiler to deal with the condensate. Also a stack size of 75mm can be used if the WC with an outlet of 80mm or less is used.
the waste from the washing machine can be solved by removing the T section and replacing it with a 90 degree elbow, then remove the redundant pipework.. If the wm waste pipe from the appliance isn't long enough, there are extenders available. the waste can then join the sink waste trap inside the kitchen cupboard.
