The plug has broken leading to less than intended insulation between the live terminal and the fixing screw. Therefore the plug fails portable appliance testing and must be replaced.
I think whilst you were heavily drinking 'again' one day you thought you'd rewire all your plugs round the wrong way, in your drunken state you thought your Meter would run backwards, thus saving you money to spend on beer and woman. Shhhhtttt, just had a thought must dash.....

(1) live, neutral wrong way around. (2) outer sheaf not in grips. (3) plug case broken between live and cover screw. (4) cover missing. Anything else ????
Phil, you forgot ,,, it has a 13amp fuse in when there's only a poxy radio on't other end of the cable, so it only needs a 3 amp fuse.
I'm not taking the credit for all the answers, I just listed them, Tom sussed the wires, Owain sussed the broken case, I just noticed outer sheaf and cover missing.
